THE SURVEY
How to participate:

The survey is available until July 30, 2006.  How much time it takes you to complete the survey depends on whether you are a survivor, a family member, a caregiver or a professional, as each survey is different.  However, we expect that most people will complete the survey within 45 minutes.

You may take the survey online or download a hard copy of the survey and return to BIANS via mail. 

We will be visiting communities across Nova Scotia to meet with survivors, family members, caregivers and professionals to conduct focus groups so that we can discuss the issues with you.  If you are interested in participating in a focus group, please contact Nancy at bianssurvey@gmail.com or (902) 473-5199.

Why you should participate:

By participating in this survey, you will have the opportunity to voice your concerns, opinions, views, and thoughts on issues facing all those who have been affected by a brain injury, whether you are a survivor, a family member, a caregiver or a professional. 

As a token of our appreciation for your input and support of this very important project, we are pleased to offer a number of prizes, kindly donated by businesses across Nova Scotia.  To be entered into the prize draw, just fill out that section of the survey.

Your responses will be held in strictest confidence
We have no way of linking your name to your responses
You are free to quit the survey at any time
You are not required to answer questions you are not
comfortable with

Results

Results of the survey will be made available through our website by September 2006, if not earlier.  If you would like to receive updates about the project, send an email to bianssurvey@gmail.com.
